Summary
A Flight Training Adelaide Diamond DA42 experienced a left engine failure on initial climb after take-off from Parafield. The instructor took control, feathered the left propeller, and returned for a circuit to a single-engine landing. Both the instructor and student were uninjured.
Investigation Findings (4)
- 01A blocked fuel injector in the Austro AE300 engine was identified as the cause of the engine failure.
- 02Blockage was caused by contaminant particles from a batch of Jet A-1 supplied at Parafield on the previous day.
- 03Fuel from the suspect batch was tested and found to contain particles at the borderline of the specification limit.
- 04Flight Training Adelaide implemented a secondary fuel filter screen in its refuelling system.
